Box Breathing: A Complete Guide to the 4-4-4-4 Technique
Inhale four. Hold four. Exhale four. Hold four. Repeat.
Box breathing — also called square breathing or four-square breathing — is one of the simplest breathing patterns there is: four equal sides, like tracing the edges of a box. It's used by everyone from Navy SEALs to nurses to people sitting in a parking lot before a hard conversation.
What box breathing is
Box breathing is a paced-breathing pattern with four equal phases:
- Inhale through the nose for a count of 4
- Hold with lungs full for a count of 4
- Exhale through the mouth or nose for a count of 4
- Hold with lungs empty for a count of 4
One full cycle takes 16 counts. At a comfortable counting pace, that works out to roughly four breaths per minute — much slower than the 12–20 breaths per minute most adults take at rest.
What's happening in your body
Slowing the breath changes what the nervous system is doing, and the mechanisms are reasonably well described in the research literature:
- Vagal activity and heart-rate variability. Slow, deliberate breathing increases activity in the vagus nerve — the main pathway of the parasympathetic ("rest and digest") branch of the autonomic nervous system. One measurable signature of this is heart-rate variability (HRV): the natural speeding-up of the heart on the inhale and slowing on the exhale becomes more pronounced at slow breathing rates (Lehrer & Gevirtz 2014).
- The exhale is the lever. Heart rate slows during exhalation, which is one reason extended or unhurried exhales feature in so many traditions. Slow breathing techniques appear to shift autonomic balance toward parasympathetic dominance through vagally mediated pathways (Russo et al. 2017).
- Brain and body talk both ways. A systematic review of slow-breathing studies found that breathing at slow paces is associated with increased HRV, increased alpha activity in EEG readings, and changes in brain regions involved in emotion and interoception — the sense of what's happening inside your own body (Zaccaro et al. 2018).
- The holds add structure, not magic. The pauses in box breathing give the pattern its even, meditative rhythm and slow the overall breath rate. Most of the studied physiology comes from the slow rate itself.
How to do it, step by step
- Sit or lie somewhere you won't be interrupted. Loosen your shoulders. You can close your eyes or soften your gaze.
- Exhale fully to empty your lungs and start the pattern clean.
- Inhale through your nose, counting slowly to 4. Let the belly expand rather than lifting the shoulders.
- Hold for 4. Keep the hold soft — no clamping the throat, no straining.
- Exhale for 4. Smooth and steady, like fogging a mirror.
- Hold empty for 4. Stay relaxed; the next inhale is coming.
- Repeat for 4–5 cycles to start. A few minutes is plenty; you can build up with practice.
If a 4-count feels like a stretch, start with 3-3-3-3. The evenness matters more than the number.

Box breathing has a steady, structured feel that suits moments when the mind is busy:
- Before a presentation, interview, or difficult conversation
- Between meetings, as a reset
- In the car (parked) after a long day
- While waiting — for results, for a call, for boarding
- Before bed, when the day won't stop replaying
Safety note
Stop and breathe normally if you feel dizzy, lightheaded, or short of breath. Skip the breath-holds — or check with your doctor first — if you're pregnant or have a heart condition, blood pressure concerns, or a respiratory condition such as asthma or COPD.
